Please note that this is not an automatic scheduling process. Once we receive your form submission, we will contact you to confirm an appointment that is as close to your requested date and time as possible.

Our form should only be used to request appointments that will take place at least 2 full business days after the form’s submission. To request an earlier appointment, please contact us directly.

If your appointment has not been confirmed within 24 hours of submitting the form, please contact our clinic by phone to confirm. We are open Monday – Friday from 7:30 am to 6:00 pm. Our clinic is closed on Saturday and Sunday.

If this is your pet’s first visit to our clinic, please also fill out our New Client Information form before the appointment.
